I was wondering if you could help guide me to what condition(s) may cause my 24hr urine VMA w/metanephrines results to be low? My results were 23 for the normetanephrines (reference range 110-1050) and 6 for the metanephrines (reference range 35-460). My doctor performed this test as a long shot for pheochromocytoma because I've had 4 ER visits in the past 6 months for sudden onset of tachycardia, headache, dizziness, nausea/vomiting, arm pain, chest tightness, elevated BP and then finally, extreme fatigue after the episodes. The ER always dismisses me as having had a panic attack, but the amazing thing is, these "attacks" wake me up from sleep and are not something I have when I'm awake. \
Of note, I'm a 43 yo female with a history of asthma and PCOS and I'm scheduled for surgery to remove a complex cyst on my ovary in 2 weeks. My complete cardiac work-up, MRA and CT of the brain were all normal.
Would you please offer any suggestions you may have as to the cause of the low test results and any diagnostic tests you think I should request from my PCP?
