Levels for total T3 and T4 measure both thyroid hormone and the proteins that carry it through the blood. In this form, thyroid hormone is inactive. Tests for free T3 and free T4 (FT4 and FT3) levels better represent thyroid status.
Levels of T4 and T3 can be low when binding protein levels are low. Binding proteins are highly influenced by estrogens and the total levels can vary during the menstrual cycle. Considering that her TSH is normal, I wouldn't be concerned about this result. If your daughter has repeat labs at some point, you might suggest that the doctor order FT4 and FT3 levels. Best, Elaine
